What type of requests does AdventHealth consider?

The program guidelines provide our criteria for sponsorship request. These request should:

  • Aim to improve access to health care services in the communities we serve
  • Advance medical or health care knowledge
  • Enhance the health and wellness of the communities we serve throughout Volusia and Flagler counties.
  • Align with the AdventHealth mission, vision and goals.

How do I submit my request?

After you have completed the sponsorship application, proceed to the final step and click "Submit" Please review your request carefully before submitting it – all fields must be completed in order to review and consider your request timely. You will receive a confirmation e-mail upon successful submission of the form.

What form(s) should are needed in order to process my request?

The W-9 and 501(C) 3 certificate or proof of non-profit status is needed.

What is the status of my request?

You will receive a letter informing you of the status of your request within 30 days.

When can I expect to receive funding?

After your sponsorship application has been approved and you have been notified, it is your responsibility to submit an official invoice with proof of your non-profit status or W-9. You can expect to receive a check within 60 days.

Can my existing request be reconsidered?

All decisions made on a specific request are final, but you can always submit a new request and refer to a previous request date. This is especially encouraged if important information has changed or a significant amount of time has passed since your initial submission.

I am not sure what hospital applies to me? How do I speak to someone directly?

Without understanding your specific needs, it is highly recommended to continue to submit your request through this system. This will be the most efficient way to route your request to the proper representative. Once the request is in the system, we will route it to the appropriate AdventHealth representative who will contact you.

What if I am requesting volunteers or help with a health fair?

There is a portion of the application that speaks specifically to this. Please make sure that you fill out the appropriate box with the specific needs you have.
